CUPS launched our newest program in 2023, Homes 4 Health (H4H), which has been instrumental in providing personalized and integrated care for individuals with complex needs that span across various areas, such as physical and mental health, addiction, economic instability, and lack of affordable housing and homelessness.
Like all CUPS teams, H4H is dedicated to addressing the full spectrum of clients' needs. This multidisciplinary team consists of Case Managers, a Housing Liaison and a Nurse, who provide intensive case management in the community to assist clients in developing a comprehensive care plan tailored to their unique circumstances.
Care plans address immediate needs, guide long-term success, empower clients to set and achieve goals, and foster readiness to access community support and services after transitioning from intensive support at CUPS. H4H also advocates for the needs of clients and supports them in navigating systems, including accessing financial support, collaborating with health professionals or landlords.
The Homes 4 Health program provides integrated, wrap-around care, and like all programs at CUPS, H4H meets individuals where they are at in their journey.
The primary goal set by H4H is to empower clients to achieve a sense of independence and the ability to one day thrive as they move forward from their journey with CUPS.
With that in mind, CUPS created a Graduation Navigator Role which has become essential to helping clients reach this readiness milestone, acting as a coach for clients as they transition from intensive support to self-sufficiency, and build their recovery capital.
Clients often build a community during their time at CUPS, and we want to support their journey while preserving this sense of community to help them thrive. With this in mind, Homes for Health Graduation Navigator Ian Potapoff created the CUPS Alumni Group, a support network for graduated clients who want to stay connected with their community and access support that matches their newfound readiness.
“The relationship does not end at the end of the client's journey,” shares Ian, “you can still come to CUPS for anything you need, and you graduate with more support, not less.”
The CUPS Alumni Group meets once per month to participate in educational, supportive, and collaborative experiences that focus on their well-being and connection with natural systems of support.
Upon graduating, clients receive peer support through these meetings, and the Graduation Navigator continues supporting clients on an as-needed basis. This support can include providing referrals to access financial support, clothing, food, medication, housing, ID, reference letters, and offering workshops on budgeting and other life skills.
Ian explains about his role, “Everything is client-driven; they are in charge of the life they want moving forward, and I am here to be a facilitator of connection, growth, community and belonging because real healing comes from connection and community; it comes from belonging.”
